Build a record around the vessel identity
A pressure vessel record should begin with stable asset identity and the current configuration. Collect design information, material records, available drawings and previous inspection reports. Confirm whether attachments and repair documents describe this vessel rather than another item in the same plant.
The pressure vessel inspection service can provide condition evidence within an agreed scope. Records support the owner's inspection programme; applicable statutory requirements and timing must be confirmed for the equipment and jurisdiction, not inferred from this article.
Locate shell, head and nozzle measurements
Tie thickness points to component regions, datums and orientation. A shell course, head or nozzle needs enough detail for a later inspector to recover the location. Include a map and photographs where they help distinguish nearby points or show the condition at measurement.
Keep local minima and uncertain readings separate from general survey values. Surface condition, temperature, coatings and method differences can affect comparison. A spreadsheet that omits point identity can lose the evidence needed to distinguish real change from a different measurement location.
Connect repair history to the affected region
Record the location and extent of repairs, replaced parts and modifications with the supporting instructions and examination records. A replacement plate or nozzle starts a different local history even if the vessel asset number remains unchanged. Do not trend its new readings as if the original metal were still present.
Preserve the original finding, repair documentation and follow-up result. Where engineering review was commissioned, link its conclusions and conditions to the repair entry. A clear repeat test does not by itself establish that every design or operating question associated with the repair has been resolved.
Keep different examination scopes distinct
External thickness measurement, internal visual examination and selected weld NDT provide different coverage. Identify which was performed, the configuration and preparation, and any areas not examined. An external survey does not describe every internal surface or concealed insulation interface.
Where reference requirements such as AS 3788 are relevant, record the agreed basis and edition in the inspection documentation. The presence of a standard in a file does not establish that all its applicable requirements were included in a particular engagement.
Make the next review easier
A useful register links each finding to evidence, responsible review, required action and close-out status. Keep service changes and unusual events with the history because they may affect future examination priorities. Retain report revisions so earlier assumptions can be understood.
Before the next inspection, reconcile the location register and repair history with the current vessel. The reviewer can then identify comparable readings, gaps and areas requiring additional evidence. Inspection records are inputs to responsible engineering and operating decisions, not automatic authorisation to continue service.
- Verified vessel identity and current component drawings.
- Recoverable thickness points with raw values and conditions.
- Located repairs, replacements and related examination records.
- Coverage exclusions, review decisions and unresolved actions.
